By signing the attached form, you give permission for your personal information, only as stated on the form, to be included in the MPQ roster and not shared outside the Guild.  Your participation in the Guild activities may result in your name appearing in the Guild newsletter and/or the website.  If you do not give permission, your name will not be included in the Guild roster and you may miss notifications regarding meeting venue changes, cancellations, etc.

JOINING/RENEWAL OPTIONS:
At MPQ meetings:  Bring your completed form and cash or cheque to the membership table.  A card will be issued on receipt of your payment.  If a receipt is required for cash, it will be provided.
By Mail:  mail your completed form and cheque to the Membership Chair.  You may pick up your card at the next meeting or have the card mailed to you.  If the card is to be mailed to you, please enclose a stamped self-addressed envelope.
